Healthcare Decision-Making: Empowering Patients to Plan Their Care

A paper with "Advance Directives Checklist" written on it, covered in spilled pills and a pen.

Patients often put off difficult care choices; sometimes until they lack the capacity to make those choices for themselves. When this happens, surrogates must step in to make decisions on the patients’ behalf. Having an advanced care plan is the most effective way to prepare patients, families and providers to make and carry out difficult end-of-life treatment decisions.

Join HANYS and MLMIC Insurance Company on June 18, 2026, at 12:00 p.m. as they discuss the use of Medical Orders for Life Sustaining Treatment in developing a care plan and address the roles and responsibilities of healthcare proxies and other decision-makers in end-of-life care.

At the end of this presentation, participants will be able to:

  • understand the types of advanced directives available to New York patients;
  • discuss the role of proxies and surrogates in healthcare decision-making;
  • describe the impact of the Medical Aid in Dying Act; and
  • empower patients to engage in advanced care planning and effectively communicate their wishes for care.

This one-hour webinar is open to HANYS members, as well as non-members.
